School health services include monitoring and supporting your child’s health and safety in the school setting throughout the year including health education and annual screenings. With your help, the school health program can enhance and maintain the highest level of health for each student and provide a rewarding school experience. Please contact us with school health questions and concerns. Remember, if your child is ill; please call your schools office to report his/her absence. Your call helps to track contagious diseases in the school community.

Health Services Forms

Health Care Provider Examination Form – Required for students entering pre-school, kindergarten, all 2nd and 5th grade students, and all new students.

Union #38 Prescription Medication Order Form – A signed doctor’s order must be on file in the health office for the administration of prescription medications during the school day. Over the Counter (OTC) medications not covered by the standing orders of our School Physician also require a signed order from a licensed prescriber.

Parent Guardian Authorization for Medication Administration – Parent authorization must be given for administration of prescribed medications or over the counter medications given at school.
