OSTP-Registration

In the menu to the left you will find the OSTP Registration Forms & Handbook necessary for the school year. The forms and parent handbook can be downloaded and printed out. Please read through all of the information and return your completed registration forms to Union #38 Out-of-School Time Programs, 113 North Main Street, Office C101, South Deerfield, MA 01373.

At the Union #38 Out-of-School Time Programs we are dedicated to meeting the needs of all our participants, providing opportunity for individual growth, and maintaining safety and enjoyment throughout their time at the program. In order to do so, we need as much information about your child as possible. Please take the time to fill out both the parent/child questionnaire and registration form completely and feel free to call with any questions or concerns.

YOUR REGISTRATION WILL BE PROCESSED IF THE FOLLOWING ARE TRUE:

• You have completed all sections of the registration form, including health care info, at least two emergency contacts, and parent/guardian signatures.
• You have enclosed the $40 registration fee. (If applicable) Registration fee and tuition can be paid by check or online with UniPay Gold. Please make the check out to the town of your child’s school. (Example. Town of Conway)
  You have completed an original registration form for each child you wish to register.

Once your registration has been processed you will receive a confirmation packet consisting of a letter confirming your child’s schedule of attendance, and several additional items to completed and returned prior to the start of the school year.

• Field Trip Passport/Emergency Card
• Behavioral Contract
• Homework Contract
• Financial aid information is available upon request.
